Notice to Clients about Consumer Rights
As professionals, clinicians at Restore Psychotherapy & Wellness, PLLC, seek to provide the best therapeutic care and regularly engage in training, education, and consultation. In addition, we welcome client questions, concerns, complaints, and feedback as this addresses client care and strengthens the therapeutic relationship. If you have any concerns, please reach out to your clinician by phone, email, or our company Contact Form, and you will be contacted as soon as possible.

Consumer Rights & Important Information
In accordance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health and Safety Code, consumers have the right to access their health care records and to file complaints regarding health care services.
Requesting Your Health Care Records
You may request copies of your health care records directly from our office. Requests must be submitted in writing. Please contact your clinician or our office directly for the appropriate authorization form and instructions.

Filing a Consumer Complaint
